Previously, ESAs were protected under the Air Carrier Access Act (ACAA) and were allowed to fly for free in airplane cabins in order to support their handler. However, the ACAA was revised, and as of January 2021, ESAs are no longer guaranteed airline cabin access.
Instead, it is up to individual airlines whether or not to consider an ESA medically necessary or simply an animal companion. Most airlines have opted for the latter, and now charge pet fees to fly with your ESA, if they allow it at all.
ESAs are also not guaranteed access to other public forms of travel, like taxis, trains, or buses. However, if you train your ESA to perform specific tasks and serve as a psychiatric service dog (PSD), they will be allowed to fly for free in an airline cabin with you, and to accompany you on any other public transportation option as well.
